By accessing or using the services provided by Corporación Las Vegas ("we," "our," or "us"), you ("Patient," "Client," or "you") agree to be bound by these Terms of Service. These terms apply to all healthcare services, website usage, and interactions with our organization. If you do not agree with these terms, please do not use our services.
2. Healthcare Services
Professional Standards
Our healthcare professionals are licensed, certified, and maintain the highest standards of care. All services are provided under physician orders and in accordance with established care plans developed specifically for each patient's needs.
Service Limitations
Our services are limited to home health care and do not include emergency medical services, hospital care, or services outside our licensed scope of practice. In case of medical emergencies, patients should call 911 or visit the nearest emergency room.
3. Patient Rights and Responsibilities
Patient Rights
- Receive quality care regardless of race, color, religion, sex, national origin, disability, or source of payment
- Be treated with dignity, respect, and consideration
- Participate in the development and modification of your care plan
- Receive information about your condition, treatment options, and prognosis
- Access your medical records in accordance with HIPAA regulations
- File complaints about care, treatment, or services received
- Privacy and confidentiality of all personal and health information
Patient Responsibilities
- Provide accurate and complete information about your health condition and medical history
- Follow the treatment plan prescribed by your physician and care team
- Inform us of changes in your condition or any concerns about your care
- Treat our staff with respect and maintain a safe environment for care delivery
- Provide timely payment for services or cooperate with insurance processing
- Keep scheduled appointments and provide reasonable notice of cancellations
4. HIPAA Privacy and Security
We are committed to protecting your protected health information (PHI) in accordance with the Health Insurance Portability and Accountability Act (HIPAA) and other applicable privacy laws.
Information Use and Disclosure
We may use and disclose your PHI for treatment, payment, and healthcare operations without your written authorization. For other uses and disclosures, we will obtain your written consent unless otherwise permitted or required by law.
Security Measures
We implement appropriate administrative, physical, and technical safeguards to protect your PHI against unauthorized access, use, or disclosure. Our staff receives regular training on privacy and security requirements.
5. Payment and Insurance
Insurance Coverage
We accept Medicare, Medicaid, and most private insurance plans. We will verify your insurance benefits and work directly with your insurance provider for payment processing. You are responsible for any deductibles, co-payments, or services not covered by your insurance.
Payment Terms
Payment is due at the time services are rendered or as agreed upon in your payment arrangement. We reserve the right to discontinue services for non-payment after appropriate notice, except in emergency situations where discontinuation would endanger your health.
6. Service Availability and Scheduling
We strive to provide services as scheduled and will make every effort to accommodate your needs. However, service availability may be affected by factors beyond our control, including staff illness, weather conditions, or other emergencies.
Cancellation Policy: Please provide at least 24 hours notice for appointment cancellations when possible. Repeated no-shows or last-minute cancellations may result in service limitations or discharge from our program.
7. Quality Assurance and Complaints
We are committed to providing high-quality care and continuously improving our services. We welcome feedback and take all complaints seriously.
Complaint Process
If you have concerns about your care or services, please contact us immediately. We will investigate all complaints promptly and work to resolve issues to your satisfaction. You also have the right to file complaints with regulatory agencies, including your state health department and Medicare.
8. Liability and Risk
Professional Liability
We maintain professional liability insurance and are committed to providing care that meets professional standards. Our liability is limited to the scope of services we provide and does not extend to medical conditions or treatments outside our control.
Environmental Safety
While we strive to provide safe care in your home environment, we cannot control all environmental factors. Patients and families share responsibility for maintaining a safe environment for care delivery, including proper lighting, clear pathways, and secure storage of medications and supplies.
9. Discharge and Service Termination
Services may be discontinued when:
- Goals of care have been achieved or services are no longer medically necessary
- Patient requests discharge from services
- Patient's condition requires a higher level of care
- Patient or family creates an unsafe environment for staff
- Non-compliance with treatment plans or failure to pay for services
- Services are no longer covered by insurance or other payment sources
We will provide appropriate notice of discharge and assist with transition planning to ensure continuity of care.
10. Website Terms
Website Usage
Our website is provided for informational purposes only. Information on the website should not be considered medical advice and does not replace consultation with qualified healthcare professionals.
Intellectual Property
All content on our website, including text, images, logos, and designs, is protected by copyright and other intellectual property laws. Unauthorized use or reproduction is prohibited.
11. Changes to Terms
We reserve the right to modify these Terms of Service at any time. Updated terms will be posted on our website with the revised effective date. Continued use of our services after changes constitutes acceptance of the modified terms. Material changes affecting healthcare services will be communicated directly to active patients.
12. Governing Law
These Terms of Service are governed by the laws of Puerto Rico and applicable federal regulations. Any disputes arising from these terms or our services will be resolved in the appropriate courts of Puerto Rico.
